Tennessee dredge company cited for a dozen health & safety violations

OSHA has issued 12 safety and health violations to Dredge & Marine Co. LLC of Millersburg, Tenn. The citations were handed down from an investigation following a July fire that killed one worker and severely injured another. A spark from a light ignited paint vapors inside a pontoon dredge compartment that had been painted. The company is owned by John W. McDougall Co.

Two safety violations were connected to inadequate exhaust ventilation and failure to use explosion-proof lighting.

Five safety violations were related to the following: use of an extension cord missing a ground prong; fire hazards from using a non-explosion proof fan with non-ferrous blades; failure to ensure certain tools were secured to prevent sparks; failure to provide fall protection; and failure to prevent bonding of metallic parts and spray guns.

Five health violations were related to the following: failure to test oxygen content before sending workers into a confined space; failure to properly train workers to work in a confined space; not having a qualified rescue team on hand; failure to continuously monitor the atmosphere in a confined space; and failure to monitor the space was safe to enter after spray painting.

The proposed citation penalties are $46,000.
