An official probe blames a faulty design and trapped piece of pipe for the failure of the blowout preventer that led to the Deepwater Horizon explosion. While the 551-page investigative report released Wednesday by the Norwegian firm Det Norske Veritas does not specifically point the finger at any company in particular, it's a fact that Cameron owned the device and Transocean maintained it. The report says the pipe stopped the blind shear rams from sealing off the well; rams are used to cut through pipe and form a seal. The April 20 blowout triggered the explosion that killed 11 workers aboard the platform and caused the BP oil spill. Cameron, Transocean nor BP commented on the report.
